Blunt trauma to the dorsal of the second metacarpal often results in a sagittal band injury that does not heal spontaneously. Consequently, scar tissue and joint stiffness develop, which are typically unresponsive to traditional therapy. This podcast explores the Casting Motion to Mobilise Stiffness (CMMS) technique, which was used to regain motion, reduce scar tissue, and rebuild strength. We delve into the assessment analysis and clinical reasoning that led to the specific cast design. A detailed demonstration on creating an intrinsic plus cast is provided, followed by the prescribed exercises to ensure the active range of motion is restored and a normal movement pattern is achieved. Notably, this patient achieved full recovery in a very short period. The CMMS technique proves highly effective in the presence of scarring and loss of range of motion with an adapted movement pattern. Its early application ensures that progress is not hindered.
